Banana Republic Credit Card Application is a GE Money Bank initiative. You must submit duly completed form to GE Money Bank, PO Box 981419, El Paso, TX 79998 for its processing and approval. This one page form has three main sections and your inputs including the signature in each section is mandatory.
You authorize GE Money Bank to contact you on all phone numbers and email address provided in the form. Read the provisions made under Sec. 766.59, Wis. Stats., or a court decree under Sec. 766.70, Wis. Stats., and provide name and address of spouse in case you are residing in Wisconsin State.

You must provide your information as the primary applicant in part 1 of Banana Republic Credit Card Application. Enter your first and last name along with an initial. Then enter mother’s maiden name and specify if you are GAP Inc., employee. Provide your home address along with street number and name, apartment number, city name, state, and zip code. Specify if your residence is own or rented. Continue by providing your date of birth, SSN, and home, business, cell phone number with area code. Select the right income bracket by marking your response in the box that applies to your annual income. Optionally, enter your email address in the next box to receive 100 bonus points, offers, and updates from Banana Republic.
Authorized User
Use this section of Banana Republic Credit Card Application form to provide an additional card to your dependant. However, you are liable to make payments towards the expenses made using this card. Enter the first and last name along with initial and select the relationship with you. You may specify the relation in case you select other.
Signature
The last section 3 of the Banana Republic Credit Card Application form is undertaking signed by you in acceptance. Please sign on the line reserved for the same and then enter the date next to the signature to complete this application form.
