I-134 Affidavit of Support Form

The I-134 Affidavit of Support Form is used when you want to want to show and agree that you are ready to financially support any relative who is not an U.S citizen and is visiting the country. The affidavit also has sections which make you take responsibility for the visiting person so that he does not become a public charge in the United States of America.

for Food Stamps, Supplemental Security Income, or Temporary Assistance to Needy Families; and
c. If the person(s) named in item 3 does apply for Food Stamps, Supplemental Security Income, or Temporary Assistance for Needy Families,
