California Adopt 200 Adoption Request Form is submitted to the Judicial Council of California for requesting adoption of a child. However, this form is suitable for requesting adoption of only one child. You must file separate Adopt 200 form for each child.

Before proceeding
- You may need to provide additional forms like ADOPT-220 and ADOPT-310 when applicable in addition to the ADOPT-200 form.
- Please complete all fields accurately and legibly.
- Do not make any entry in the field reserved for the entry by the Court’s clerk.
- Provide your name and case number on the foremost portion of page 2 and 3 of the ADOPT-200 form.

How to Fill the California Adopt 200 Adoption Request Form
Part 1: Enter your name as adopting parent, then relationship with the child, and address with phone number. Provide details of lawyer representing the case if any.
Part 2: Select only one option to specify the type of adoption from 7 options offered.
Part 3: Provide child’s new name, gender, date of birth, age, address, place of birth, child’s consent for adoption if above 12 years, and date of possession of the child.
Part 4: Enter child’s name before adoption.
Part 5: Select if the child has a legal guardian, enter the date of the guardianship order, county, and case number.
Part 6: Select if child is court’s dependant and then enter Juvenile case number and name of county.
Part 7: Select if child has Indian ancestry and enclose ADOPT-220 form if you select yes.
Part 8: Enter name of child’s birth parents if known.
Part 9: Complete 9.a, 9.b, and 9.c in case of adoption by agency.
Part 10: Complete 10.a, 10.b, and 10.c in case of independent adoption.
Part 11: Complete 11.a, 11.b, and 11.c in case of stepparent adoption.
Part 12: Select part 12 if child is conceived by artificial insemination.
Part 13: Select most suitable choice among the available for contact after adoption. You may need to enclose ADOPT-310 when applicable.
Part 14: Select your response for the consent of the presumed father and/or birth mother when applicable according to Fam. Code, § 8606 subdivision.
Part 15: Provide name of parent, relationship with the child, and date if the court terminated parental rights.
Part 16: Provide name, relationship with the child, and date if the child is subjected to tribal customary adoption order as per Welf & Inst. Code, § 366.24.
Part 17: Enter name and relationship to child if Petition to Terminate Parental Rights is filed along with California Adopt 200 Adoption Request Form.
Part 18: Enter name and relationship to the child of persons not contacted for one year or more when Application for Freedom from Parental Custody is filed as per (Family Code, § 8604(b).)
Part 19: Enter the name and relationship of the dead parent with parental rights.
Part 21: Select your response between the two options about the legal relationship of adopting parent/s with the child and tribal customary adoption.
Part 22: Enter date and your printed name, along with the signature of the representing attorney if any.
Part 23: Complete California Adopt 200 Adoption Request Form by entering date and printed name along with signature of adopting parent. Use separate line for name and signature of each adopting parent.
