District of California Bankruptcy Proof of Claim Form B10 serves the purpose of providing the bankruptcy claim value as of the date of filing the claim. This eight-part form requires information pertaining to the claim and filling all parts of the form is compulsory. Please provide descriptive information about the basis for claim in part 2 and secured claim in part 4.
