11. INFORMATION ABOUT SPOUSAL MAINTENANCE (ALIMONY) REQUEST (if applicable)

Neither party is entitled to spousal maintenance (alimony).

Husband/Wife is entitled to spousal maintenance because: lack sufficient property to provide for
my reasonable needs; am unable to support myself through appropriate employment; am the
custodian of a child or children whose age or condition is such that I should not be required to seek
employment outside the home; lack earning ability in the labor market adequate to support myself;

REQUESTS I MAKE TO THE COURT:

(HUSBAND/WIFE) asks the court to order the following:

1. DISSOLUTION (DIVORCE): Dissolve the parties’ marriage and return each party to the status of a
single person;

3. SPOUSAL MAINTENANCE (ALIMONY): Order spousal maintenance to be paid by the
husband/wife in the amount of per month, beginning with the first day of the month after
the signing of the Decree of Dissolution and continuing until the person receiving the spousal
maintenance is remarried or upon the death of either party, or
for months.

4. CHILD SUPPORT: Order that child support shall be paid by husband wife in a reasonable
amount as determined by the Court under the Arizona Child Support Guidelines and consistent with
Arizona law.

5. INSURANCE AND HEALTH CARE EXPENSES FOR CHILDREN: Order that
husband wife shall pay for health, medical, dental insurance coverage for unemancipated
children, and that the husband wife shall pay for all reasonable unreimbursed medical,
dental, health-related expenses incurred for the child(ren) in proportion to their respective incomes
as described on the Proposed Child Support Order, which shall be submitted with the decree,
shared equally, other.

6. TAX EXEMPTION: I request that the Court allocate the right to claim the child(ren) as a dependent(s)
for income tax purposes according to the Arizona child support guidelines.

7. CHILD CUSTODY AND PARENTING TIME: Award custody and parenting time of the children
under 18 years as follows (check one alternative).

A. SOLE CUSTODY: Sole custody of the minor child(ren) awarded to husband/ wife subject to
parenting time as follows:

(1) Reasonable parenting time rights to the parent not having custody, consistent with any
existing county access guidelines or as ordered by the court.

(2) Supervised parenting time. Supervised parenting time between the child(ren) and the
husband/wife according to law.

B. JOINT CUSTODY: Joint Custody – husband and wife agree to share custody of the children,
an set forth in the Joint Custody Agreement by the parties pursuant to Arizona law, signed by both
parties, if the Court adopts the agreed terms of the Joint custody Agreement setting forth the custody
and parenting time agreements between the parties. There have been no significant acts of Domestic
Violence as described by Arizona law.

8. SEPARATE PROPERTY: Assign each party his or her separate property

9. COMMUNITY PROPERTY: Make a fair division of all community property as requested in this
Petition.

10. COMMUNITY DEBTS: Order each party to pay community debt as requested in the Petition, and order
each party to pay any and all other community debts unknown to the other party.
